진행성유방암 환자에서의 Alpelisib 투여를 위한 동반진단검사
PIK3CA 유전자는 cell signaling pathway에 관여하는 인산화효소 중 하나인 phosphatidylinositol 3-kinase (PI3K)의 alpha isoform (p110α)을 코딩하는 유전자이다. PIK3CA 유전자의 변이가 발생할 경우, downstream pathway의 항산성활성화(constitutive activation)을 유발함으로써, 세포성장/분열, 단백 번역/ 합성, 고사조절(regulation of apoptosis) 등의 장애를 초래하게 되며, 유방암의 경우, 내분비치료에 대한 저항성(resistance to endocrine therapy) 및 질병진행(disease progression)과 연관이 있는 것으로 알려져 있다.

PIK3CA 유전자 변이는 colon adenocarcinoma, endometrial adenocarcinoma, lung adenocarcinoma, breast invasive lobular carcinoma 등 다양한 암종에서 관찰된다. 유방암의 약 70%에 해당하는 환자가 hormone receptor (HR) (+), human epidermal growth factor receptor 2 (HER2) (-)을 나타내며, 이 중 약 40%에서 PIK3CA mutation을 가지는 것으로 알려져 있으며, PIK3CA mutation의 종류/빈도 및 위치는 다음과 같다.

PIK3CA-mutated cancer cohort(median follow-up: 20 months)에서의 무진행생존기간(progression-free survival, PFS)을 비교하면, alpelisib–fulvestrant 병용 치료군은 11.0 개월 (95% confidence interval [CI], 7.5 to 14.5), placebo–fulvestrant 치료군은 5.7 개월 (95% CI, 3.7 to 7.4) (hazard ratio for progression or death, 0.65; 95% CI, 0.50 to 0.85; P<0.001)로 확인되었다.

PIK3CA mutation군에서 알펠리십(alpelisib, Piqray®) 투여에 따른 반응정도 및 무진행생존기간(PFS)이 높게 보고되어 치료결과와 관련성이 있다. 또한 검체(조직 및 혈장) 간의 일치도는 수용 가능한 수준으로 유효한 기술로 인정됨에 따라, 유방 종양 조직 또는 혈장에 대한 PIK3CA 동반진단 검사가 2021년 신의료기술(보건복지부고시 제2021 – 197호)로 고시되었다. 진행성유방암 환자에서 폐경 후 여성 및 남성 중 HR (+), HER2 (-)인 환자를 대상으로 치료약제(alpelisib) 투여를 위한 환자를 선별하는데 있어 PIK3CA 동반진단검사를 활용하실 수 있게 되었다.
혈장에서의 양성율은 disease burden, non-tumor shedding/clearance 등에 영향을 받을 수 있다. 따라서, 조직과 혈장 검체간의 불일치(discrepancy)는 tumor heterogeneity, different techniques, spatial/temporal factors, plasma DNA로 오염될 가능성 등으로 설명되고 있으며, 혈장에서 PIK3CA mutation이 음성인 경우, 조직 검체에 대한 PIK3CA mutation 확인이 권고된다.

동반진단검사로 허가된 QIAGEN Therascreen PIK3CA RGF PCR kit는 PIK3CA 유전자 중 Exon 7, 9, 20에 위치하는 11개의 주요 변이를 확인할 수 있다.

결과해석 시 유의사항
PIK3CA 동반진단검사로 허가된 Therascreen PIK3CA RGQ PCR Kit의 제조사인 QIAGEN의 Urgent field safety notice (첨부파일 참조)에 따르면, Q546R 양성변이의 위양성 결과가 확인됨에 따라, 제조사가 해당 문제점을 해결할 때까지 Q546R은 보고되지 않음을 참고하시기 바라며, 결과지에는 아래의 설명을 추가할 예정이다.
*The Q546R mutation is not reported according to manufacturer’s instructions, because therascreen PIK3CA RGQ PCR Kit may generate false Q546R mutation positive results caused by non-specific molecular interactions within the Q546R reaction (To be resolved by Qiagen).
